[Conservative methods for diagnosing catheter-associated bacteremia].

چکیده
Catheter-associated bacteremia (CAB) is a frequent cause of nosocomial infection in the critical patient and implies an increase in both morbidity--mortality and healthcare costs. The classical method for confirming CAB involves the concomitant isolation of the microorganism in blood cultures obtained by percutaneous puncture and from catheter tip cultures. This conventional procedure has the inconvenience of requiring catheter withdrawal in order to allow tip culture. In this context, there are arguments both in favor and against systematic catheter removal when suspecting CAB. In favor of withdrawal is the fact that many studies have reported a lesser mortality or duration of CAB when the catheter is removed. However, these studies pose the limitation of having a non-randomized design. In turn, the arguments against catheter withdrawal include: (I) the low yield of systematic catheter tip culture, with positive cultures in under 10% of all cases according to different series; (II) a randomized study has shown that routine catheter removal is not necessary in stable patients. The study included patients with suspected CAB, and excluded hemodynamically unstable subjects, immune depressed patients and individuals with signs of local infection. The patients were randomized to either routine catheter removal or catheter maintenance until the

متن کاملCloxacillin as an antibiotic lock solution for prevention of catheter-associated infection.
INTRODUCTION Catheter-related infection is associated with increased all-cause mortality and morbidity in hemodialysis patients. This study aimed to evaluate an antimicrobial lock solution (cloxacillin and heparin) in temporary noncuffed double-lumen catheters for long-term intermittent hemodialysis as a method of preventing catheter-related infection. MATERIALS AND METHODS Patients on hemodi...
متن کاملKlebsiella pneumoniae bacteremia associated with a Tesio hemodialysis catheter.
Klebsiella pneumoniae bacteremia associated with a Tesio hemodialysis catheter To the Editor: Catheter-associated bacteremia is a serious complication of temporary hemodialysis vascular access. Bacteremia is the second leading cause of death in hemodialysis patients. Infectious episodes are also the leading cause for untimely catheter removal and for morbidity in dialysis patients. In a study o...
متن کاملIs Taurolidine-citrate an effective and cost-effective hemodialysis cathe-ter lock solution? A systematic review and cost- effectiveness analysis
Background: Prevention of catheter-related infection is of prime importance,. However, because of the risks caused by the leakage of circulating antibiotics and development of resistance to antibiotics, they are replaced by lock solutions. The aim of this study was to evaluate the efficacy and cost- effectiveness of taurolidine-citrate as a hemodialysis catheter lock solution compared to other ...
متن کاملValidity of earlier positivity of central venous blood cultures in comparison with peripheral blood cultures for diagnosing catheter-related bacteremia in cancer patients.
We carried out a prospective study in two French Comprehensive Cancer Centers (95 and 184 beds, respectively) to assess the validity of a test based on the earlier positivity of central venous blood cultures in comparison with peripheral blood cultures for predicting catheter-related bacteremia. The differences between the times to positivity for the 21 patients with clinical catheter-related b...
